Job Description

For over 50 years, Thresholds has provided progressive community mental health treatment aimed at helping individuals define their personal recovery paths and live independent and fulfilling lives. Thresholds is seeking social-justice minded individuals with exceptional interpersonal skills to:
 
 promote the physical and emotional well-being of Thresholds’ members by teaching them independent living skills and fostering a therapeutic environment through the use of problem-solving, coaching, and relationship-building techniques.
 
ESSENTIAL DUTIES & RESPONSIBILITIES:
                Provide direct supervision for members in the Residences.
Coach members in independent living skills, such as housekeeping, shopping, meal preparation, personal hygiene and activity planning.
                Teach and monitor appropriate social skills amongst members. Assist members in building strong emotional functioning. Co-lead groups and activities on a regular basis.
                Engage with members constantly around their goals, interests, and skill building.
                Manages emergencies and crises within milieu utilizing TCI skills.
                Keeps the residence neat and clean at all times. Advises Shift Supervisor, Team Leader, or designee of any maintenance problems and issues. Responsible for the creation of a safe, clean, interesting, and inviting atmosphere for members.
                Performs and participates in crisis intervention in accordance to TCI and Thresholds standards. Models best practices for crisis intervention as trained in TCI (Therapeutic Crisis Intervention) to other staff. Performs physical management when necessary.
                Remains on work-site and on-duty until relief coverage is engaged; or until proper coverage is arranged and with supervisor approval, per guidelines established by the program. Maintains proper residential coverage at all times.
                Provides accurate and timely daily documentation commensurate with Medicaid and DCFS standards. This paperwork includes, but is not limited to shifts summaries, group and individual notes, DCFS documentation, accident/incident reports, communication logs, and data reporting to Quality or Research Departments and other documentation as required by the program.
                Ensures compliance with Thresholds members’ Residential agreements.
                Implements, in a positive, professional manner, ongoing programming and policy changes as described in agency memoranda and supervisory directives.
                Monitors Self Administration of Medication. This includes all procedures associated with the documentation of medication monitoring of self-administration in keeping with Thresholds standards, as well as procedures to follow in the event that there is any disruption to the medication schedule.
                Communicates significant information regarding house members to other staff workers through the staff log. Ensures that all topics and issues related to the operation of the agency and the well-being of its members are addressed. This includes practicing appropriate utilization of the off-hours on-call system for questions and emergencies.

MARGINAL DUTIES & RESPONSIBILITIES:
May be required to work flexible schedules and be available to work on weekends or evenings as required.
                May be required to conduct apartment searches, as needed. May be required to assist in hospitalization procedures when members are in need of psychiatric or medical hospitalization.
