Job Description

For over 50 years, Thresholds has provided progressive community mental health treatment aimed at helping individuals define their personal recovery paths and live independent and fulfilling lives. Thresholds is seeking social-justice minded individuals with exceptional interpersonal skills to:
promote the physical and emotional well-being of Thresholds’ members by teaching them independent living skills and fostering a therapeutic environment through the use of problem-solving, coaching, and relationship-building techniques as well as to supervise shift staff at Thresholds’ residential facilities.
ESSENTIAL DUTIES & RESPONSIBILITIES
Assumes the leadership role within the program’s residence.
Coordinates proper coverage and oversees the daily operations for the Program’s residences.
Mentors and advises House Managers in decision making regarding appropriate responses to members’ needs, including regular supervision with direct reports.
Conducts weekly house meetings with members, and initiates evening activities to promote socialization. This may include integration of members into community by taking them on outings and teaching them to use public transportation.
Performs and participates in crisis intervention in accordance to TCI and Thresholds standards. Models best practices for crisis intervention as trained in TCI (Therapeutic Crisis Intervention) to other staff. Performs and supervises physical management when necessary.
Teaches members independent living skills, such as housekeeping, shopping, meal preparation and personal hygiene.
Maintains the security of the residence by locking the residence upon leaving and enforcing proper procedures with regard to money, medication, and possessions of members and the agency.
Maintains proper residential coverage at all times.
Oversees and maintains the safety of members, staff and visitors. This requires the upkeep of fire and safety equipment and document accordingly. Conducts all safety drills in conformance with current CARF and Agency standards and complete documentation as instructed.
Keeps the residence neat and clean at all times. Advises Team Leader, or designee of any maintenance problems and issues. Responsible for the creation of a safe, clean, interesting, and inviting atmosphere for members
Ensures compliance with Thresholds members’ Residential Agreements and all Thresholds policies and procedures. This may require confronting members’ inappropriate behavior, providing corrective action, and teaching alternative responses in accordance to the TCI training.
Communicates significant information regarding house members to other staff workers through the staff log. Ensures that all topics and issues related to the operation of the agency and the well-being of its members are addressed. This includes practicing appropriate utilization of the off-hours on-call system for questions and emergencies.
Implements, in a positive, professional manner, on-going programming and policy changes as described in agency memoranda and supervisory directives.
Schedules and attends weekly treatment team meetings, and assists in formulation and evaluation of members' treatment plans.
Monitors Self Administration of Medication. This includes all procedures associated with the documentation of medication monitoring of self-administration in keeping with Thresholds standards, as well as procedures to follow in the event that there is any disruption to the medication schedule.
Finishes all assigned paperwork completely and on time. This paperwork includes shift summaries and notes, DCFS 906 documentation and Accident/Incident reports. This paperwork may also include but is not limited to reporting data to Quality or Research Departments, medication monitoring, accident/incident, occupancy reports, service notes, group attendance, process notes and other notes as required by the program.

MARGINAL DUTIES & RESPONSIBILITIES
May be required to work flexible schedules and be available to work on weekends or evenings as required. May be required to be on call.
May be required to maintain the household petty cash fund.
